Important Membership Info
· Membership is a full calendar year from first payment
· Payments due 1st of month
· Must give 30 days written notice of early termination
· $395 early cancellation fee that is non-negotiable
· Automatic Withdrawal monthly from credit card
· Must sign agreement
· Any unused treatments at the end of the calendar year are forfeited and do not roll over
· Cannot gift treatment to someone else
· If doing monthly installments cannot get filler for 4 months
· If you chose to not get a filler with your package then can substitute for a skin treatment of their choice or 25 units of Botox
